ABSTRACT

Abstract Objective: The objective of this project is to describe the epidemiological profile of the Contact Lens Sector of the Benjamin Constant Institute in Rio de Janeiro. Methods: A retrospective study of 788 cases was carried out from March / 2004 to June / 2015. Results: Most of the objects in the cases were female (62.23%). As for the indication, 445 cases (56.47%) were optical, 281 cases (35.65%) were esthetic, 44 cases (5.58%) were prosthetic, and 18 cases (2.30%) were cosmetic. The most prevalent age group was from 20-39 years of age (46.34% of cases), followed by 40-64 years of age (25.31% of cases). Of the total of adapted cases, 81.6% were with rigid lenses, and among the optical medical indications, 61.4% of the cases were by keratoconus. Conclusions: Because it is a reference medical service, whose main objective is the rehabilitation of patients with low vision, the optical medical indication was the most frequent. The rigid lenses showed up with a prominent role in the adaptations.


Resumo Objetivo: Descrever as estatísticas do Setor de Lente de Contato do Instituto Benjamin Constant do Rio de Janeiro. Métodos: Estudo retrospectivo de 794 casos no período de março/2004 a junho/2015. Resultados: A maior parte dos casos foi do sexo feminino (62,21%) e a indicação mais frequente foi médicas ópticas (56,04%). A faixa etária mais prevalente foi a de 20-39 anos, correspondendo a 46,34%, e em seguida a de 40-64 anos (25,31%). Do total de casos adaptados, 81,6% foram com lentes rígidas. Entre as indicações médicas ópticas 61,4% dos casos foram por ceratocone. Conclusões: Por ser tratar em um serviço médico de referencia, cujo principal objetivo é a reabilitação de pacientes com visão subnormal, a indicação médica óptica foi a mais frequente. As lentes rígidas mostraram-se com papel de destaque nas adaptações.

RESUMO Objetivos: Avaliar o perfil demográfico e comportamental de pacientes com indicação médica óptica de lentes de contato (LC) em um hospital público de Curitiba. Métodos: Estudo transversal realizado através de questionário em pacientes atendidos no serviço de lentes de contato do ambulatório do SUS do Hospital de Olhos do Paraná, no período de 02 de setembro de 2014 a 04 de agosto de 2015. Resultados: Dos 69 entrevistados, 56,52% fizeram LC. Destes, 55,07% têm entre 20-49 anos, 69,56% são mulheres, 40,58% com ensino médio completo e 36,23% com 2 a 6 salários mínimos. Predominaram a lente rígida com 66,67%, com menos de 8 horas de uso em 41,02% e desconforto durante o uso em 59%. A limpeza das LC é feita ao colocar e ao retirar em 87,18% dos casos, com 71,8% utilizando solução própria para LC. Alto custo da compra foi o principal motivo do não uso de LC, com 70% dos pacientes que não fizeram uso desse recurso. Conclusões: Dos entrevistados, 56,52% iniciaram o uso de LC, sendo o ceratocone a doença mais indicada. Foi demonstrado nesse estudo que os pacientes em questão têm boa orientação quanto a manuseio e manutenção das LC. Apesar dos inquestionáveis avanços da tecnologia médica, continuam ocorrendo a desistência do uso desse recurso principalmente pelo custo e medo do manuseio. É responsabilidade do especialista esclarecer os benefícios que os pacientes com indicação médica terão com o uso de LC, assim como motivar o seu uso.


ABSTRACT Objectives: To assess the demographic and behavioral profile of patients with optic medical indication of contact lenses (CL) in a public hospital in Curitiba. Methods: Cross-sectional questionnaire study of patients who attended the contact lens service of the national health system (SUS) outpatient clinic from the "Hospital de Olhos do Paraná" between September 2nd, 2014 and August 4th, 2015. Results: Of the 69 people interviewed, 56.52% acquired CL. Of these, 55.07% are between 20-49 years old, 69.56% are women, 40.58% had finished high school and 36.23% earned a salary of 2 to 6 times the minimum wage. Rigid lenses prevailed, representing 66.67% of the total, with less than 8 hours of usage in 41.02% of the cases and discomfort during usage was assessed in 59%. The cleaning of CL was done at the moment of insertion and removal in 87.18% of cases, and 71.8% used contact solution. High purchase cost was the main reason for not using CL for 70% of patients who did not use this resource. Conclusions: Of the patients interviewed, 56.52 % started using LC. Keratoconus was the main disease for which CL were indicated. It was demonstrated in this study that patients in question have good guidance on handling and maintenance of LC. Despite the unquestionable advances in medical technology continue to occur the abandonment of the use of this feature mainly the cost and fear of handling. It is the responsibility of the expert clarify the benefits that patients with medical indication will have with the use of LC, as well as encourage their use.

ABSTRACT

PURPOSE: To determine effectiveness of laser-assisted in situ keratomileusis (LASIK) in the treatment of astigmatism following penetrating keratoplasty (PK). METHODS: We performed a retrospective review of medical records of patients who underwent LASIK following PK and had over 1 year of follow-up data. RESULTS: Twenty-six patients (26 pairs of eyes) underwent LASIK following PK. Mean age of the patients at the time of LASIK was 40.7 years (range, 26 to 72 years). Following LASIK, the mean cylinder was reduced by 2.4 diopters and mean reduction of cylinder after LASIK was 65.4% from the preoperative values at the last follow-up visit. Uncorrected visual acuity became 20 / 50 or better in 69.2% of the eyes after LASIK. Best-corrected visual acuity became 20 / 50 or better in 73.1% of the eyes after LASIK. All of them were intolerable to contact lenses before LASIK. After LASIK, 6 pairs (23.1%) did not need to use contact lenses and 18 pairs (69.2%) were tolerable to using contact lenses or spectacles. There were no significant endothelial cell density changes 12 months after LASIK (p = 0.239). CONCLUSIONS: LASIK is effective in the treatment of astigmatism following PK and increases contact lens and spectacle tolerance.

ABSTRACT

Context: Boston ocular surface prosthesis (BOSP) is a scleral contact lens used in the management of patients who are rigid gas permeable (RGP) failures as with corneal ectasias such as keratoconus and in those patients who have ocular surface disease such as Stevens–Johnson syndrome (SJS). Aim: To report utilization of BOSP in a tertiary eye care center in India. Materials and Methods: We retrospectively reviewed charts of 32 patients who received BOSP from July 2008 to May 2009. Indications for fitting these lenses, improvement in visual acuity (VA) before and after lens fitting and relief of symptoms of pain and photophobia were noted. Paired t-test was used for statistical analysis using SPSS version 16.0 for Windows. Results: Thirty-two patients (43 eyes) received these lenses. These consisted of 23 eyes of 17 patients who failed RGP trials for irregular astigmatism and corneal ectasia such as keratoconus and post radial keratotomy and scar and 20 eyes of 15 patients with SJS. Mean age of RGP failures was 27.94 years. Pre- and post-BOSP wear mean LogMAR VA was 1.13 and 0.29, respectively, in RGP failures. The P value was statistically significant (P < 0.001). In patients with SJS, LogMAR VA was 0.84 ± 0.92 before and 0.56 ± 0.89 after lens wear. The P value was statistically significant (P < 0.001). VA improved by >2 lines in 7/20 eyes (35%) with SJS, with improvement in symptoms. Conclusion: BOSP improves VA in patients who have irregular astigmatism as in ectasias and RGP failures and improves vision and symptoms in patients with SJS.

ABSTRACT

The tear film is a fluid that covers the cornea which establishes a high quality refractive surface for the corneal epithelium. Here we investigated the alterations in tear film feming due to contact lens wear and compared it with normal and artificial tear ferning. The tear samples were collected from 4 normal individuals and 4 contact lens users aged from 22 to 34 years. The tear samples were pulled out from the eye by a small duct and observed under light microscopes. Artificial tears 'Refresh Plus', from 'Allercan' were purchased from a shop. In artificial tears, the structure of ferning in the centre and periphery were similar to each other under light microscope where as in normal tears, peripheral ferning was mildly denser compared to the centre. In the contact lens user tear film, ferning was faint under the light microscope. The ferning of tear film of was in the form of circle in the centre and a string of circles at periphery. There was minor difference between normal and artificial tear ferning. The contact lens user tear ferning was very thin compared to normal and artificial tear ferning. This could be due to less lipid content in the tears of contact lens user. The contact lens use affects the tear ferning. This could be due to loss of lipid content in the tears especially in the central region

ABSTRACT

PURPOSE: To describe etiologies and clinical characteristics of corneal opacities leading patients to seek cosmetic treatments. METHODS: The medical records of 401 patients who presented for cosmetic improvement in corneal opacities between May 2004 and July 2007 were retrospectively reviewed. The following parameters were analyzed: age, gender, cause of corneal opacity, time course of the corneal disease, associated diseases, prior and current cosmetic treatments, visual acuity, location and depth of the corneal opacity, and the presence of either corneal neovascularization or band keratopathy. A single practitioner examined all patients. RESULTS: The most common causes of corneal opacity were ocular trauma (203 eyes, 50.6%), retinal disease (62 eyes, 15.5%), measles (38 eyes, 9.5%), and congenital etiologies (22 eyes, 5.5%). Prior treatments included iris colored contact lenses (125 eyes, 31.1%) and corneal tattooing (34 eyes, 8.46%). A total of 321 of 401 eyes underwent cosmetic treatment for corneal opacities. The most common treatment performed after the primary visit was corneal tattooing (261 eyes, 64.92%). CONCLUSIONS: This is the first study to investigate the causes and clinical characteristics of patients presenting for cosmetic treatment of corneal opacities rather than for functional improvement. Various cosmetic interventions are available for patients with corneal opacities, and these should be individualized for the needs of each patient.

ABSTRACT

The use of contact lens (CL) for the correction of refractive errors, cosmetic use and their usage as a therapeutic modality for corneal pathologies has increased tremendously over the years. The present study was conducted with the aim to find a pattern of CL use amongst college students with a focus on the rationale for CL use and problems related to their use. This study includes 371 college students who were current users of CL at the time of the study. Results showed that 96.8% of the CL users use the ‘daily wear type’ of CL. Most quoted reasons of usage were comfort and convenience (61.2%) with cosmetic benefit (42.9%) as the next most common reply. Common complaints were that of general discomfort (foreign body sensation), dry eyes and watering eyes. Educated use of CLs amongst its users is advised in view of the symptoms and associated complications that may occur.

INTRODUÇÃO: O ceratocone é ectasia corneal progressiva e não inflamatória, geralmente bilateral, que resulta em baixa visão devido ao astigmatismo irregular. O impacto da doença sobre a qualidade de vida depende não somente dos seus fatores limitantes, mas também da personalidade do indivíduo. OBJETIVO: Verificar a qualidade de vida e os estilos de personalidade dos pacientes com ceratocone. MÉTODOS: Um grupo de 68 pacientes com ceratocone e um grupo de referência para controle com 52 pessoas emétropes foram avaliadas. Foram analisados dados pessoais, dados referentes ao ceratocone, exame oftalmológico, aspectos da qualidade de vida por meio do questionário genérico SF-36 e inventário Millon de estilos de personalidade (MIPS). Ambos os questionários foram de autopreenchimento. RESULTADOS: Os dois grupos eram homogêneos na avaliação dos dados pessoais. Analisando o questionário SF-36, foram observadas pontuações mais baixas para o grupo de pessoas com ceratocone, tanto no componente físico (353,0 ± 58,2 x 379,1 ± 52,9) (p=0,005) como no mental (326,3 ± 89,9 x 364,2 ± 66,4) (p=0,004), principalmente para a capacidade funcional (86,3 ± 9,7 x 92,6 ± 9,7) (p <0,0001) e para o estado geral de saúde (76,8 ± 16,9 x 83,5 ± 16,5) (p=0,014). Quanto ao MIPS, verificou-se diferença significante entre os grupos visto que as pessoas com ceratocone se apresentam mais pessimistas (p=0,007), intuitivas (p=0,004), retraídas (p=0,014) e inseguras (p=0,010). CONCLUSÃO: Os resultados demonstram alterações quanto aos aspectos psicossociais nos pacientes com ceratocone. Houve influência na qualidade de vida, piorando tanto o componente físico quanto o mental revelado pelo SF-36. Os pacientes com ceratocone demonstraram ser mais pessimistas, intuitivos, retraídos e inseguros que o grupo controle pelo inventário MIPS.


INTRODUCTION: Keratoconus is a progressive and non-inflammatory ectasia of the cornea, mostly bilateral causing low vision because of the irregular astigmatism. The influence of the disease on quality of life is dependent on limiting factors and personality type. PURPOSE: To verify quality of life and personality type of the person with keratoconus. METHODS: Sixty-eight individuals with keratoconus and 52 without the ocular disease were studied. The following data were collected: personal demographic data, keratoconus measurements and specific questions, complete ophthalmologic examination, measurement of quality of life and personality type using standardized instruments. RESULTS: Both groups were demographically similar and comparable. SF-36 and MIPS differed between keratoconus and control patients. SF-36 scores showed that the keratoconus group had lower scores regarding the physical component (353.0 ± 58.2 x 379.1 ± 52.9) (p=0.005) as well as the mental component (326.3 ± 89.9 x 364.2 ± 66.4) (p=0.004), mainly functional capacity (86.3 ± 9.7 x 92.6 ± 9.7) (p<0.0001) and general health (76.8 ± 16.9 x 83.5 ± 16.5) (p=0.014). MIPS scores showed that patients with keratoconus were significantly more pain avoiding (p=0.007) presented more, imaginative intuition (p=0.004), asocial withdrawal (p=0.014) and anxious hesitation (p=0.010) than the control group. CONCLUSION: The results showed psychosocial impairment in the keratoconus group. Keratoconus influenced the quality of life expressed by the SF-36. MIPS showed keratoconus patients to be more pain avoiding, with more imaginative intuition, asocial withdrawal and anxious hesitation than the control group.

OBJECTIVE: To verify age of first ophthalmic evaluation and optical prescription along with present optical correction among ophthalmologists of different age groups. METHODS: A cross-sectional study was conducted with a nonprobabilistic sample (n = 578) of ophthalmologists and resident physicians using a self-administered questionnaire. RESULTS: The first ophthalmologic exam was undergone before age 7 for 33.3 percent of the ophthalmologists aged between 23 and 30 years, for 25.8 percent of those aged from 31 to 42 , and for 15.8 percent of those aged from 43 to 76 years (P < 0.0005). The first ophthalmologic exam was undergone at 8 to 22 years of age for 60.7 percent of the ophthalmologists aged between 23 and 30, for 54.9 percent of those aged between 31 and 42, and for 47.4 percent of those aged from 43 to 76 years of age. Use of the first optical prescription (eyeglasses) between 0 and 7 years was reported by 10.0 percent, from 8 to 20 years of age by 59.6 percent, from 21 to 40 years by 18.1 percent, and 41 years or older by 12.3 percent . Present use of optical correction was reported by 69.0 percent. Concerning type of corrective device chosen, 63.7 percent wore only eyeglasses, 29.8 percent wore eyeglasses and contact lenses, and 6.5 percent wore contact lenses only. CONCLUSION: Among the sample of opthalmologists, the first ophthalmic evaluation and corresponding optical correction usually occurred relatively late (8 to 20 years of age). However, in the younger group of ophthalmologists, there was a highly significant increase in the number of subjects who had undergone an ophthalmologic exam before age 7. Eyeglasses were reported as the correction of choice by those in all age groups.


OBJETIVO: Verificar em oftalmologistas de diferentes faixas etárias a idade do primeiro exame oftalmológico, dos primeiros óculos e tipos e correlação óptica em uso, a fim de substituir estudos sobre a evolução dos cuidados oftalmológicos nas últimas décadas. MÉTODOS: Foi realizado estudo transversal em amostra prontamente acessível formada por oftalmologistas e residentes (n = 578), por meio da aplicação de questionário. RESULTADOS: Submeteram-se ao primeiro exame oftalmológico até os 7 anos de idade, 33,3 por cento dos oftalmologistas na faixa etária de 23 a 30 anos; 25,8 por cento na faixa etária de 31 a 42 anos e 15,8 por cento de 43 a 76 anos (teste exato de Fischer P < 0,0005). O primeiro exame oftalmológico ocorreu entre 8 e 20 anos de idade em 60,7 por cento dos oftalmologistas na faixa etária de 23 a 30 anos; 54,9 por cento na faixa etária de 31 a 42 anos e 47,4 por cento na de 43 a 76 anos. Manifestaram terem usado os primeiros óculos entre 0 e 7 anos 10,0 por cento, entre 8 e 20 anos 59,6 por cento, entre 21 e 40, 18,1 por cento; e de 41 anos ou mais, 12,3 por cento. Dos 69,0 por cento que mencionaram uso de correção óptica, 63,7 por cento usavam apenas óculos, 29,8 por cento intercalavam o uso de óculos e de lentes de contato e 6,5 por cento usavam apenas lentes de contato. CONCLUSÕES: A época do primeiro exame oftalmológico e da correção óptica ocorreu tardiamente (entre 8 e 20 anos). Porém na faixa etária mais jovem houve um aumento altamente significativo dos indivíduos submetidos a exeme oftalmológico até os 7 anos de idade. Registrou-se preferência por uso de óculos em todos os grupos etários.

OBJETIVO: Analisar o perfil dos usuários de lentes de contato entre os estudantes de medicina da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista - UNESP - Botucatu (SP), quanto à ametropia, à aquisição, aos cuidados e às complicações. MÉTODOS: Foi aplicado um questionário aos alunos do 1° ao 6° ano de Medicina da Faculdade de Medicina de Botucatu da Universidade Estadual Paulista - UNESP - Botucatu (SP), perfazendo um estudo analítico transversal. RESULTADOS: Foram 566 entrevistados. Destes, 155 (27,4 por cento) usavam lente de contato. Das lentes de contato usadas, 94,8 por cento eram gelatinosas, e em 98,7 por cento o grau era negativo. O descarte dessas lentes era mensal em 46,9 por cento dos entrevistados e 63,9 por cento a utilizavam, em média, 14 horas/dia. Faziam aquisição da lente de contato em óptica ou farmácia 14,1 por cento dos estudantes. Faziam a higiene ao colocá-las e ao retirá-las dos olhos, 46,5 por cento. As complicações referentes aos olhos vermelhos eram 51,0 por cento; 44,5 por cento, embaçamento; e 42,6 por cento, sensação de corpo estranho, além de 4 estudantes relatarem "ceratite". CONCLUSÕES: A maioria dos alunos utilizava lentes hidrofílicas, descarte mensal e grau negativo. A adaptação inadequada, aquisição de lentes de contato em ópticas ou farmácias e complicações graves ocorreram, mesmo nos usuários socioculturalmente considerados diferenciados. Em vista disso, demanda a extrema necessidade de maior atenção na abordagem do tema em questão, a fim de prevenir o comprometimento visual permanente.

ABSTRACT

Realizou-se estudo clínico prospectivo e randomizado para avaliar a adaptação de lentes de contato rígidas gás-permeáveis monocurvas e bicurvas (desenho Soper-McGuire) em 126 pacientes portadores de ceratocone. O tipo de adaptação inicial mais freqüente foi em três toques, seguido pelo toque apical. Os dois desenhos causaram aplanamento das medidas ceratométricas mais curvas, reduziram a toricidade corneal e o componente esférico do erro refrativo. A porcentagem de sobrevida no uso de lentes monocurvas foi de 60,32 por cento e no de bicurvas, 71, 43 por cento. Este estudo mostrou que um único tipo de lente não proporciona adaptação ideal para todos os casos.A prospective and randomized clinical trial was conducted to evaluate monocurve and bicurve (Soper-McGuire design) RGP contact lenses fitting in 126 keratoconus patients. The most frequent initial fitting was in three touches, followed by the apical touch. The two designs caused flattening of the steep keratometric curve, reducing corneal toricity and the spherical component of the refractive error. The survival rate for monocurve lenses was 60.32 per cent and 71.43 per cent for bicurve lenses. ABSTRACT

Objetivo: Este estudo buscou verificar se a prescriçäo adequada de lentes corretoras pode ser realizada exclusivamente com os dados fornecidos pela refraçäo automática objetiva. Métodos: Todos os pacientes foram submetidos a anamnese, exame oftalmológico. A refraçäo clínica, por meio de recursos propedêuticos clássicos näo-automatizados objetivos e subjetivos para prescriçäo de lentes corretoras ("gold standard"), seguido por exame no refrator automático TOPCO N KR3000. Resultados: Foram estudados 1001 olhos de 504 pacientes, dos quais 45,2 por cento, do sexo masculino. A média de idade foi de 36,6 anos. O índice geral de concordância de diagnóstico entre refraçäo clínica e refraçäo automática objetiva foi de 66,7 por cento .Considerando-se tolerância de -0,50 a +0,50 DE, o índice de concordância quanto ao componente esférico foi de cerca de 90 por cento .Houve concordância em 27,60 por cento dos astigmatismos hipermetrópicos e miópicos simples e de 97 ,7 por cento nos astigmatismos compostos e no astigmatismo misto. A cicloplegia näo alterou de maneira estatisticamente significante o índice de concordância de diagnóstico. O eixo das lentes cilíndricas indicado pela refraçäo automática objetiva apresentou proximidade estatisticamente significante ao eixo da refraçäo clínica. Conclusäo : A refraçäo automática objetiva fornece dados úteis para a prescriçäo de lentes corretoras, desde que se levem em consideraçäo variáveis como uso prévio ou näo de óculos, idade e cicloplegia. A prescriçäo de lentes corretoras näo pode ser realizada exclusivamente com dados fornecidos pela refraçäo automática objetiva.

ABSTRACT

El entendimiento en profundidad de los diferentes mecanismos de daño, defensa y reparación que toman lugar en una causticación ocular, ha permitido tratar en mejor forma a estos pacientes. En la etapa aguda, la medida más importante continúa siendo la irrigación profusa del ojo en el mismo lugar del accidente. Lisis de adherencias, antibióticos, corticoides, hipotensores y citrato de sodio deben emplearse. En la etapa subaguda nuestro norte será promover la epitelización (lente de contacto blanda terapéutica, tarsorrafía) y, evitar y tratar la ulceración. En la etapa tardía deberemos tratar las complicaciones y uno de los procedimientos empleados es la rehabilitación de la superficie ocular, con el transplante de células epiteliales sanas; puede hacerse tan precozmente como cuatro semanas despúes de la causticación, siempre que la isquemia limbar haya pasado y el limbo se encuentre vascularizado

Uma investigaçäo sobre as condiçöes de uso de correçäo óptica mostrou que 49,5 por cento dos funcionários e 56,7 por cento dos estudantes da UNICAMP, com idades entre 18 e 39 anos, usam correçäo óptica. Deste total, 76,6 por cento dos funcionários e 71,7 por cento dos estudantes usam óculos 23,4 por cento e 28,3 por cento, respectivamente, usam lentes de contato. 41,6 por cento dos funcionários e 33,9 por cento dos estudantes usuários de óculos, gostariam de mudar o tipo de correçäo óptica para lentes de contato
